Model Call Giveaway Questionnaire

Thank you for your interest in my model call giveaway! I’m looking for the perfect fit, and this form will help me get to know you and your family better. Please answer as honestly and fully as possible—there are no wrong answers!

📸 Important Notes:

  • The session will take place in my studio, and participants must wear outfits from my curated studio wardrobe.
  • Professional hair and makeup (included in the giveaway) will be done by my makeup artist at her studio in Fridley before the session.
  • The session will be offered on week days only between 10am & 2pm and session must be completed BEFORE May 2025.
  • You will receive all the edited files from your session, as a thank you.
  • A signed model release is required so I (and my hair and makeup artist) can share images from the session.
